Your skin is not a sterile shell. It is a living surface, home to billions of microorganisms that work alongside your own cells to defend, balance and protect you.

This invisible community, known as your skin microbiome, shapes how resilient your skin feels, how it reacts to products and weather, and how readily it recovers from treatment. When it is balanced, your skin looks calm and behaves predictably. When it is disrupted, problems tend to follow.

What is the skin microbiome?

A microbiome is a community of microscopic life that shares a single environment. Your skin, the largest organ in your body, hosts one of the busiest microbiomes you have. It is made up of bacteria, fungi, viruses and tiny mites, living across the surface and down inside your pores and hair follicles.

Most of these organisms are harmless, and many are actively helpful. They are not passengers. They occupy space, consume resources and release compounds that influence the skin around them, which is precisely why they matter to how your skin looks and feels.

Your skin microbiome is as individual as a fingerprint. It begins forming at birth and is then shaped by your genetics, your environment, your hormones and the products you apply day after day.


Why is the skin microbiome important?

The relationship between you and your skin microbes is largely mutual. You give them a stable place to live and a steady supply of nutrients in the form of oils and dead skin cells. In return, beneficial microbes help hold your barrier together, keep more aggressive organisms in check and quietly train your immune system to tell friend from foe.

Think of your skin as a walled garden. When the soil is rich and the right plants are thriving, weeds struggle to take hold and the whole plot looks healthy. Strip the soil bare, and opportunists move in.

That balance is why two people can use the same product and get opposite results, and why skin that is stripped or over-treated often becomes more reactive, not less. Protecting the microbiome is one of the most overlooked foundations of healthy, resilient skin, and it sits at the centre of how we approach regenerative skin treatment at the clinic.

What does your skin microbiome do?

Your skin microbiome works alongside several of your body's defence systems. Some of these roles are well established, others are still being mapped by researchers, but the picture that emerges is of an ecosystem that is far more active than a simple coating of germs.


Barrier defence

Helpful microbes physically occupy the space that troublesome ones would otherwise colonise, a process sometimes called competitive exclusion. They also produce their own antimicrobial substances that suppress harmful bacteria, effectively acting as a first line of defence before your immune system is even involved. A strong, well-populated barrier loses less water and stays more comfortable.


Immune training


Your skin is a major immune organ, and its resident microbes help calibrate that immune response from infancy onwards. They teach immune cells which organisms are safe to ignore, which reduces unnecessary inflammation. A microbiome that is too sparse or unbalanced is thought to contribute to skin that overreacts to harmless triggers.


Acid mantle and pH


Healthy skin sits at a slightly acidic pH, usually around 4.5 to 5.5. Beneficial microbes both depend on and reinforce this acidity, which in turn discourages many harmful species that prefer a more alkaline surface. Harsh, high-pH cleansers can tip this balance and make the surface friendlier to the wrong organisms.


Inflammation control


Certain skin microbes release compounds that calm inflammatory signalling, while an overgrowth of less desirable species tends to drive it up. Chronic, low-grade inflammation is linked to redness, sensitivity and accelerated visible ageing, which is one reason a settled microbiome is associated with calmer, healthier-looking skin.

Where is your skin microbiome?

Your microbiome is spread across your entire skin surface, but it is far from uniform. Different regions behave like different habitats, each suiting different organisms.

  • Oily areas such as the face, scalp, chest and back are rich in sebum and favour oil-loving species, including bacteria from the Cutibacterium family.

  • Moist areas such as the folds of the elbows, behind the knees and the underarms suit organisms that thrive in warmth and humidity.

  • Dry areas such as the forearms and lower legs are more exposed and support the most varied mix of species.

Microbes live not only on the visible surface but also deep within pores, hair follicles and sweat glands. These sheltered spaces act as reservoirs, which is why the microbiome can repopulate the surface after cleansing. It also explains why treatments that work at the level of the follicle, including some forms of CO2 laser resurfacing, need careful aftercare to support recovery.

What is skin dysbiosis?

Dysbiosis is the term for an unbalanced microbiome. On the skin it generally means one of three things, and in practice these tend to overlap:

  1. A loss of helpful, protective microbes.

  2. An overgrowth of potentially harmful species.

  3. A loss of the healthy diversity and balance between species.

Once the balance tips, the skin's defences weaken, which makes it easier for the wrong organisms to dominate and harder for the helpful ones to recover. This is why irritated skin can feel as though it is caught in a cycle.



What factors affect your skin microbiome?


Over-cleansing and harsh products

Frequent washing with stripping, high-pH cleansers removes protective oils and disrupts the acid mantle. Aggressive scrubs, high concentrations of active ingredients used too often, and alcohol-heavy formulas can all thin out the beneficial population and leave the barrier exposed.


Antibiotics and medication

Antibiotics, whether taken by mouth or applied to the skin, do not distinguish between helpful and harmful bacteria. They can clear an immediate problem while also depleting protective species, which is part of why some skin conditions return once treatment stops.


Environment and lifestyle

Ultraviolet exposure, pollution, climate, stress, sleep and diet all influence the surface environment your microbes live in. Smoking and a diet high in sugar and processed foods are associated with a less favourable balance, while UV exposure both damages skin cells and alters which organisms can thrive.


Age and hormones

The microbiome shifts across your life. Hormonal changes in puberty, pregnancy and menopause change sebum production and surface chemistry, which in turn reshapes the community of microbes your skin supports.


What conditions relate to skin dysbiosis?


An unbalanced microbiome is associated, to varying degrees, with several common skin conditions. Research is ongoing, and the microbiome is rarely the only cause, but the links are increasingly recognised:

  • Acne, in which an overgrowth of certain strains within oily, inflamed pores plays a role. Gut balance can also play a part, through the gut-skin axis.

  • Eczema and other forms of atopic, barrier-impaired skin, where reduced diversity is commonly seen.

  • Rosacea, which has been linked to changes in surface organisms, including skin mites.

  • Seborrhoeic dermatitis and dandruff, associated with an overgrowth of yeast in oily areas.

  • Folliculitis and recurrent surface infections, where protective competition has broken down.

  • Sensitive, reactive skin that flushes, stings or flares without an obvious cause.


What are the signs of an unbalanced skin microbiome?


There is no single test that confirms dysbiosis, but skin often signals when its balance is off. Common signs include:

  • Persistent redness or flushing.

  • New or increasing sensitivity and stinging from products you once tolerated.

  • Tightness, flaking or a barrier that never quite feels comfortable.

  • Breakouts that do not settle with usual care.

  • Dullness and a loss of the smooth, even surface of healthy skin.


Can I test my skin microbiome?


Several companies now sell swab kits that claim to analyse your skin microbiome at home. While the science is fascinating, it is not yet developed enough for these reports to guide personalised treatment in a reliable way. We currently learn far more from a careful clinical assessment of your barrier, hydration, sensitivity and history than from a consumer swab. As research advances, this is likely to change.

What can I do to look after my skin microbiome?


Much of the work happens at home, and the principles are reassuringly simple:

  • Cleanse gently. Use a mild, low-pH cleanser once or twice a day, and resist the urge to scrub or over-wash.

  • Protect the barrier. Moisturise to support the skin's lipids, and use daily sun protection to limit UV damage.

  • Introduce actives slowly. Powerful ingredients have their place, but using too many, too often, is a common cause of reactive skin.

  • Consider prebiotic and postbiotic skincare. Formulas designed to feed or mimic beneficial microbes show early promise, though evidence is still developing.

  • Support skin from the inside. A varied, whole-food diet, good sleep and managed stress all help the surface environment.

  • Use antibiotics carefully. Take them only when genuinely needed, and follow your clinician's guidance on restoring the skin afterwards.

The most resilient, healthy-looking skin we see is rarely the most heavily treated. It is the skin whose barrier and microbiome have been respected. That belief shapes how we treat every patient who walks through our doors in Chelsea.



Frequently asked questions


Can I test my skin microbiome?

Consumer swab kits exist, but the science is not yet mature enough for the results to guide personalised treatment reliably. We assess the skin clinically instead, looking at barrier function, hydration, sensitivity and the pattern of any breakouts.


Does washing my face too much damage my microbiome?

It can. Frequent washing with harsh, high-pH cleansers strips protective oils and raises skin pH, which favours less helpful microbes and weakens the barrier. A gentle, low-pH cleanser once or twice a day is usually enough.


Are probiotic skincare products worth using?

Some show early promise, particularly prebiotic and postbiotic formulas that feed or mimic beneficial microbes. The evidence is still developing, so we recommend choosing well-formulated products and prioritising barrier support over marketing claims.


Can in-clinic treatments harm the skin microbiome?

Well-planned treatments respect the barrier. We tailor protocols and aftercare so that resurfacing, injectables and regenerative treatments support recovery rather than leaving the skin raw or over-stripped.
